A variant of the flag can be seen outside the city building. This variant uses text, with the city name in the ywllow field. On the blue wave is the state name.

According to NAVA-News IX:4, the yellow area and wavy bars symbolize the
sunny rolling hills and streams of Middletown. As for the canton, it suggests a
sign hanging before a tavern. Along with the key, they both suggest Middletown's
heritage as a midway stop for travelers in the early days. The designer is Dick
Crossett, who was a free-lance advertising designer in Louisville, KY and
certainly a vexillologist. Sadly, no date of adoption is mentioned in the
article, but it is almost for certain sometime in the very late years of the
'60s or the early '70s. Would like to note, he did not use text on his design,
so it is certain that a city government member made this addition.
